    Here is a lot of toy medical forms from the Tinytown Medical Kit by Peerless Playthings. c1948. The original medical kit included all kinds of doctor and nurse toys. These items are the paper forms that accompanied the physical toys. Included in this lot are: 4 Red & white Doctor appointment forms; 6 Visiting nurse appointment forms; an Eye Chart; 2 warning plaques; 3 certificates of health (2 different sizes and colors); a sick chart from Tinytown hospital, a prescription form; and rules for the visiting nurse. 19 items altogether. A few had pencil names that were poorly erased. Other were never used. These are great if you have the kit and not the forms. Sold as a lot only.
